Supporting LA

Living Skills for Those in Need

Developing generic & natural unpaid circles of support for clients

  • Encouraging clients to maintain relationships with family and friends.
  • Supporting clients to look for social/recreational opportunities in their communities.
  • Supporting clients by assisting to look for and apply for resources (i.e. food banks, public services, Metro/ACCESS, IHSS, SSI/SSA/SSDI) available to them.

Assists clients to implement choices about their home environment

  • The location of his or her home

  • The kind of home desired

  • Others who may live there

  • How the home is set up and how it looks

Informing and supporting every client regarding choices in service delivery by

  • Acknowledging that each person can choose their service provider which may not be UP-LIFT.
  • Ensuring that each client participates in selecting the UP-LIFT staff who will provide support.
  • Informing clients that they can request a change in service provider if they feel the person is not meeting their needs.
  • Ensuring the client receives services in a time and place convenient for them
  • Ensuring the client decides who will be part of their circle of support. UP-LIFT’s role is to assist the client in finding and strengthening the support circle towards meeting their personal goals.

Assisting and supporting the client in structuring and maintaining the following

  • An environment within the home typical of other homes in the community.

  • A community-oriented life in which all generic and natural supports are accessed consistently with the needs and preferences of the client.

  • A network of critical and durable relationships with others, including a circle of support consisting of a majority of members who are not paid to support the client, and with appropriate family participation.

  • An environment where they are able to form and maintain friendships and relationships with others.

  • Within the parameters of supported living regulations and the service design, UP-LIFT is responsive to daily decisions about what the client needs or wants from UP-LIFT.
